Why Was Lindsay Clancy's Medication Increased The Day Before?

The day before Lindsay Clancy killed her three children, her medication was increased. Nobody has fully explained why that decision was made, or by whom. 
She'd already been on roughly thirteen different psychiatric drugs across about four months. Her own malpractice lawsuit describes that final dose increase, paired with a night without sleep, in the hours before Cora, five, Dawson, three, and Callan, eight months, died in their Duxbury, Massachusetts home. She's pleading not guilty by reason of insanity.
